Blood Collection Protocol

Patient Verification
Confirm patient identity using two unique identifiers (e.g., full name and medical ID number) to prevent sample mix-ups and ensure result accuracy.

Site Preparation
Cleanse the venipuncture area (e.g., antecubital vein) with 70% alcohol or chlorhexidine using a circular motion. Allow the site to air-dry to maximize antiseptic efficacy and avoid contamination.

Needle Insertion
Anchor the vein and insert a sterile needle at a 15–30° angle. Maintain steady pressure to achieve smooth blood flow into the collection system.

Tube Handling
Fill tubes in the recommended order of draw (e.g., blood culture → coagulation → serum tubes) to preserve additive integrity. Adhere to volume markings for precise blood-to-additive ratios.

Sample Mixing
Gently invert tubes containing additives 8–10 times (avoid shaking) to ensure homogeneous mixing and prevent clotting or hemolysis.

Labeling & Documentation
Label tubes at the bedside with:

Patient name/DOB

Unique ID number

Collection time/date

Collector's initials
Verify labels match the requisition form before transport.

Post-Procedure Care
Apply pressure to the puncture site, then bandage. Dispose of sharps in a puncture-proof container.
